THANK YOU FOR PARTICIPATING

For good and valuable consideration, the receipt and sufficiency of which being hereby acknowledged, I being competent and of legal age, hereby give L&F Distributors, LLC, Favorite Brands, LLC, and Retro Health, LLC (collectively, the “Releases”) the absolute and irrevocable and perpetual right, license and permission, with respect to my likeness, performance, and participation in its Marketing Projects and Campaigns (the “Projects”);

a) To record/photograph my voice, name, likeness, performance and participation;

b) To copyright the same in its own name or in any other name which it may choose;

c) To publicize my participation photos or any portion thereof by any means, for any purpose whatsoever in whole or in part, including (but not by way of limitation), promotion, advertising, trade; and

d) To use my name in connection therewith if it so chooses.

I agree that the foregoing grant includes the right to use my physical likeness, name, voice, performance and/or participation in any form, including a photograph, picture, artistic rendering, silhouette or other reproduction by photograph, film, tape, digitally or otherwise.

I acknowledge that the Releases is and will be the sole owner of all rights in and to the communications and the recording/photography thereof, for all purposes in perpetuity.

I hereby assign any copyright rights, publicity rights or any other rights that I may have regarding the above communications and my participation in the Projects to the Releases.

I also hereby release the Releases and its directors, officers, employees, successors and assigns from any and all claims, causes of action or demands of any nature whatsoever which I now have or which I could or might have against the Releases by reason of any fact or matter whatsoever, including but not limited to, for defamation, invasion of privacy, right of publicity or intellectual property infringement.

This instrument shall be governed by the laws of the State of Texas, without reference to its choice of law principles. Venue for any cause of action hereunder shall lie exclusively in Hidalgo County, Texas.
